This recent BBC news item pretty clearly involves a fraudulent LPA:

The OPG processes when setting up an LPA clearly involve quite a lot of box ticking, but do they, or the police, follow up when a fraudulent LPA is created?

Unless it's pretty blatant and well witnessed, I would think not. Too hard to prove and get past a jury.
